CRB/DBS Check Forms

 

From the 28 March 2013 forms with the old CRB branding on them will no longer be accepted. All DBS Checks should now be carried out using the new DBS branded forms. Any forms received by the Disclosure and Barring Service which are old CRB branded forms will be rejected from the 28 March 2013. For more information click here.
